You started HRT — but how do you know it is actually working? Why a single blood value cannot tell you, and what to do when you barely feel a difference.
Whether your HRT is working is not shown by a single number — it shows in whether your symptoms ease and how your trend develops over weeks. Symptom relief matters more than hitting a target lab value. With transdermal delivery (gel or patch), a trough level in the morning before the next dose is the most informative timing. And if you barely notice anything, the reason is rarely "too little hormone" — more often it is the application site, the route, your test timing, or ferritin and thyroid function masking the effect. You started. Gel, patch or tablet — the step you may have avoided for months is done. And then? Something happens. Or not much. Or something, but not what you were hoping for.
Sleep is a little better, mood is not. The hot flushes are gone, the brain fog stays. Or you feel — honestly — much the same as before, and quietly wonder whether you are doing something wrong.
You are not doing anything wrong. But you are asking exactly the right question. "Is this actually working?" is not an impatient question — it is the beginning of steering your therapy instead of just taking it.

What "It Is Working" Really Means
Most women start HRT and expect a blood test to say: "Yes, that fits." Unfortunately it does not work that way.
There is no single universal estradiol target above which HRT is "correctly dialled in". The reason is simple: two women with exactly the same serum value can feel completely different. One is symptom-free, the other still exhausted, anxious, awake at three in the morning.
That is why the yardstick in modern menopause care is not "which number" but "how are you". The guidance is consistent: the lowest effective dose that relieves your symptoms — not the dose that produces a particular value.
This is not an excuse to skip testing. Values are valuable — as context, not as verdict. A value tells you where you stand right now. Your symptoms over time tell you whether it is getting better. Together they form the picture.
What You Should Actually Track
If symptoms are the real yardstick, it is worth taking them seriously — not as a vague "so-so", but as something you observe across weeks.
The things that typically shift on effective HRT:
• Sleep – do you sleep through, or still wake at night • Hot flushes and night sweats – rarer, milder, or gone • Mood and inner tension – irritability, anxiousness, thin-skinned days • Mental clarity – are your words coming back • Joints and energy – morning stiffness, the afternoon crash
The trick is not to judge from one good or bad day. It is the trend. A bad night says nothing. Four better weeks say everything.

Why You May Feel Nothing — It Is Rarely "Too Little Hormone"
This is the part almost nobody explains. When HRT does not land, the answer is almost never "simply more". Usually something else is going on — and that is good news, because it is solvable.
The application site with gel. Estradiol gel is absorbed through the skin, and absorption varies — with the site, skin condition, and whether you moisturise or shower right after. Applying it in a different place and in a rush every day can make your level correspondingly unsteady.
The route. Oral estradiol passes through the liver first. This first-pass effect lowers the measurable serum level and changes what arrives in your body. Transdermal delivery bypasses it. If your values look low even though you take your tablet, that is often not an efficacy problem — it is physiology.
The missing progesterone. If you still have a uterus, estradiol needs a progestogen component — not as an afterthought, but to protect the endometrium. Unopposed estrogen is associated with an increased risk of endometrial changes (Gompel 2020). Your test timing. The classic. An estradiol value means something entirely different depending on when you test. With transdermal delivery you measure the trough — in the morning, before you apply the next dose. Test shortly after applying and you see a peak and assume all is well. Test at the wrong time and the numbers seem to make no sense — and you draw the wrong conclusions.
So before you think about a higher dose, always look here first: site, route, progesterone, timing. And talk to your doctor before changing anything.
The Silent Players: Ferritin and Thyroid
Here is something many women — and unfortunately some clinicians — overlook: a perfectly adjusted HRT can feel like a failure when something else is pulling in the background.
Low iron stores are associated with exhaustion, hair loss and concentration problems — exactly the symptoms you are taking HRT for. The same is true for the thyroid. Sluggish thyroid function overlaps symptomatically almost entirely with low estrogen — cold sensitivity, weight change, brain fog, low mood. And a "normal" TSH does not rule out suboptimal conversion.
This is why looking at hormones in isolation helps so little. Your HRT does not act in a vacuum. It acts in a body where iron, thyroid and hormones talk to each other constantly. Only when you see them side by side can you tell whether your HRT is truly not working — or whether another marker has been blocking the stage all along.
Testosterone: The Often Forgotten Axis
One thing hardly anyone talks about: women produce testosterone too, and that changes in this phase of life as well.
If your HRT has sleep and hot flushes well under control but libido, drive and a certain inner fire are missing, testosterone can be part of the picture. The evidence is clear for one area — sexual function — and considerably thinner for everything else: an international consensus statement found demonstrated benefit for low sexual desire, but insufficient evidence for other uses (Davis et al. 2019).
Staying honest means: testosterone is not a cure-all, in most countries it is not formally approved for women, and it belongs in medical hands with monitoring of values. But it is an axis worth mentioning in conversation — especially when "almost everything" is better but a piece is missing.
"Normal" Is Not "Optimal for You" — Least of All on HRT
The most frustrating sentence after a blood test is: "Everything is in range." Especially when you know something is off.

Personal Experience
I started with one pump of estradiol gel and 100 mg progesterone. The first thing I noticed — before any value confirmed it — were the hot flushes. They eased. That was the moment I thought: okay, this is really doing something.
With progesterone it was sleep. Immediately, clearly, genuinely. But I stay honest, because nobody told me this beforehand: progesterone also makes me slightly woozy. Not unpleasant — more like a gentle winding down. Wonderful if you are going to bed anyway. Unhelpful if you still had plans. So my first lesson was simple: I only take progesterone when I am home and the day is done. No more driving. And something I found out purely by observing — taken with dinner, especially with olive oil, the effect becomes quite strongly damping. Very pleasant on the way to bed. Not something to take on the go. The amount needed adjusting too: 200 mg was too much for me, 100 mg is enough — partly because my body still makes its own progesterone.
With estrogen it was a different path — more trial, more patience. My aim was an average around 90 pg/mL. With one pump I kept landing at 60, 68 — and the brain fog stayed. So I went up to two pumps, always with food. But the 90 was never the actual goal — it was my guardrail. What led was always how I felt; without the brain fog I would never have pushed the number up. The other direction mattered just as much: not too much. I wanted to stay in a sensible range and not overshoot — so: increase, then measure. A bit like a biohacker: change something, then look at what the body does.
And what I saw surprised me. Sometimes a value was suddenly 200 or 300 — and next time back at 90. My own estrogen fluctuates enormously. But you only see that if your measurements are comparable at all. So I became strict with myself: always on the same cycle day, day 3 for me. Always at the same time, between ten and eleven. And the gel always at the same time in the morning on the same arm, between seven and eight. Only then are you measuring yourself — and not coincidence.
In the beginning I measured on day 3 until it felt settled. And I still measure on day 3 now and then, when time and money allow. Two pumps are the good middle ground for me.
I am honest about the rest too: I am sensitive to PMS, and I have the impression progesterone sometimes makes that more noticeable — too much of it also makes me tired. These are exactly the points where a conversation with your doctor about alternatives pays off: a different progesterone route, a hormonal IUD for heavy bleeding, or cycle suppression for very severe PMS. There is no single solution — there is your path, found through trial, observation and time. Right now I am on two pumps and 100 mg progesterone, which I sometimes take vaginally when I want less sedation. All of it agreed with my doctor.
And that is my whole message in the end: symptoms lead. The lab helps find the limits — whether you have too little or too much. One measurement is not enough for that. Which is also why one appointment is not enough, where someone says "all good" or "all bad". The truth lies in the fluctuations — and you only see those if you look more often.
